Getting older often means taking multiple prescription medications. In fact, most people 45 and older take an average of four prescription medications each day. If you or a loved one take prescription drugs, it is important to be aware of what drugs you are taking, dosages and how certain drugs can interact with other medications.
Medication reconciliation
You can help prevent medicine errors by taking an active role. HealthEast Care System is urging all patients to bring in their prescription and over-the-counter medications (in their original bottles) to every clinic visit. This practice, known as medication reconciliation, is a way to ensure accuracy of medications and dosages, so that your clinician can provide the highest and most accurate level of care. If this is not possible, a list of your medications should be brought in to every clinic visit. Your clinician will verify the list of medications during your visit and you will leave the clinic with an updated medication list, including any changes. It is also very important that you bring medication lists to any visit you have with other health care providers, including your dentist.
Refilling your prescription
In the interest of providing the safest possible medical care for our patients, especially in the use of prescribed medications, your HealthEast Clinic asks that you follow these two very important steps when refilling your medications.
- Always call your pharmacy for a refill, even if it says no refills. This request will generate an electronic message from your pharmacy to your clinic, which will be reviewed by your clinician. Your clinician will electronically transmit the approval back to your pharmacy. This is the safest way for the clinic to monitor your medications. It decreases the possibility of errors in handwriting or verbal communication. It also ensures the correct medications and dosages are dispensed by your pharmacy.
- Please do not wait until the last minute. Although we do our best to expedite your refill the same day, approving your medication refill may take up to two business days. We require up to two business days, so that we are able to seek approval from each patient's primary physician. Please plan ahead and follow the process that has been put in place to keep you safe. Help us ensure the safest possible use of prescribed medications.
