Health care reformers are putting forth a new model for patient care: the Medical Home model.Initiated nationally by the American Academy of Pediatrics, the model recommends that each patient has a personal physician who knows his or her medical and family history and oversees the patient's medical care. The physician not only treats specific conditions, but also works with the patient to manage his or her overall health and coordinates care with other professionals. The model emphasizes prevention and improving the care of chronic conditions. Reformers believe this model of health care will improve prevention and disease management efforts, which will hold down health care costs.
