Extended Care Program:
Providing the Care You Need When and Where You Need It Most

The physicians at Talbert Medical Group think extended care is crucial, and they make it possible to treat our frail patients where they reside as well as in the doctor's office.
Starting this winter, Talbert instituted the Extended Care Program (ECP) available at no charge to all qualifying Talbert patients. This comprehensive new service extends medical care wherever our most frail patients reside: their home, nursing homes, board and care, or assisted living facilities.
TMG has dedicated a team of medical practitioners including physicians, nurse practitioners and physician assistants that care for this select group of patients. The ECP medical practitioners take the time to care for their patients not only on a medical level, but on a personal level as well. The team pictured here is headed by Talbert's Senior Medical Director, Dr. Osmundo Saguil. Melissa Medellin, N.P., one of those also pictured above, is a member of the ECP team. Melissa provides regular medical visits to frail patients in their homes. She also rounds weekly at skilled and custodial care nursing homes surrounding the TMG medical centers, taking care of Talbert patients residing in those facilities. "I enjoy seeing patients in their home environment," she says. "This allows me to get to know my patients and their families, not just treat their illness. I am able to establish close relationships with all my patients; not only as a provider but as a human being as well!"
TMG practitioners care for patients as often as needed depending on individual conditions and needs. They visit patients in nursing homes at least once a week.
